Need to cancel your membership? We can help with that. While we hate to see any of our wonderful members leave, we understand that sometimes circumstances change. Simply fill out our Membership Request Form and enjoy your final 30 days of membership - we'll do the rest.
Your 30 day Cancellation period begins only when you fill out the form below.
Centerline Martial Arts is a small local business with a small staff. In order to provide time for processing and avoid last-minute requests, we have a 30 day cancellation period for ALL memberships even if your membership is on hold -
no exceptions.
PLEASE NOTE: We do not process same-day requests.
